▣ Abstract

Quantification of the hemodynamic features of blood flow is important because these are closely related to the development of cardiovascular diseases. Blood flow generate various types of fluid dynamic stresses, such as pressure and frictional stress, on the blood vessel. Endothelial cells on the vessel wall sense and respond to these hemodynamic conditions by adapting their morphology and proliferation. The pathological hemodynamic stresses exerted on the vessel wall consequently induce inflammatory lesions in the vessel that promote the development of atherosclerosis, aortic dilatation, and aortic aneurysm. In addition, the mechanical forces of the blood flow influence plaque vulnerability and the risk of aneurysm rupture. Therefore, quantification and understanding of the characteristics of blood flow are important for diagnosing and predicting the future risk of cardiovascular disease.

Magnetic resonance imaging (MRI) techniques provide non-invasive and non-ionizing methods for the accurate anatomic depiction of the cardiovascular system. Based on the inherent flow sensitivity, MRI could be used to investigate hemodynamics features in patients with anatomical data within a single measurement. In particular, time-resolved and three-dimensional characterization of blood flow using 4D Flow MRI has achieved considerable progress in recent years. This presentation introduces the principle and procedures of 4D Flow MRI. Various fluid dynamic biomarkers for possible clinical usage are also described, including wall shear stress, turbulent kinetic energy, and relative pressure. Lastly, the presenter will discuss pitfalls of the method and remaining issues to be used for clinical routine.
