Dear GIT Students,
To proceed with the health insurance termination, please send a cancellation request email directly referring to the following information:
1. Prepare all required documents:
① Insurance Cancellation Form (attached)
② Copy of Alien registration card
③ Copy of Bank book
2. Send an email to: info@insclick.com
3. Write an email title as: “Insurance Cancellation”
* Please note that the insurance period must be remained at least 1 MONTH to Mid-contract termination (e.g. if the term of your contract is from Feb 22nd 2020 to Feb 21st 2021, you have to apply for the cancellation at least before January 20th 2021).
* Also, there must be NO claims records when submitting an application.
